Terms of Contract and Disclaimer

The English reference below sets out this Japanese-language contract and injury waiver that you must agree to train with us.　Please note that this is a non-binding reference. The terms of the Japanese-language contract and disclaimer govern our relationship.

第1条
本文章は、当道場会員（以下、甲とする）と、大賀道場（以下、乙とする）との間に交わされた同意書である。
This is the agreement between you and the Oga Dojo.

第2条
甲は乙の会則を遵守しなければならない。
You promise to abide by the rules herein.

第3条
甲は乙に入会するにあたり、別紙の入会金を支払う。
You promise to pay your training fees as set forth in the current payment schedule (provided separately)​.

第4条
甲は乙へ、練習への参加不参加を問わず、乙の定める方法で、毎月、別紙の会費を支払う。
You agree to pay the monthly training fees as set forth in the current payment schedule and acknowledge that this payment is due whether you actually train or not.

第5条
甲は以下の行為を行った場合に、乙の判断によって、乙の機関から退会となる。
(1)甲が乙の会則を遵守しなかったとき
(2)甲が乙の名誉を傷つけたとき
(3)甲が乙の機関に相応しくないと判断されたとき
You understand that your membership in the Oga Dojos may be terminated​ if, in the sole discretion of Oga Dojo, it is determined that:
(1) you have not abided by the Oga Dojo Rules;
(2) you damage the reputation of Oga Dojo; or
(3) you otherwise behave yourself in a way unacceptable to the Oga Dojo.

第6条
甲が退会を希望する場合は、前月の10日までに、乙が定める所定の手続き（(1)退会届の提出。(2)会費滞納分 の支払い）を完了させなければならない。手続きが完了し、乙が退会を認めるまでは、いかなる理由があろうとも、甲は乙に対する月ぎめの会費を支払う義務を 有する。
In the event you decide to end your membership with Oga Dojo, you must complete the prescribed procedures for ending your membership ​(​(1) provide notification of termination and (2) pay all outstanding fees owed​)​ by the 5th day of the month before your membership terminates. Until you successfully terminate your membership, you remain responsible without exception for your monthly membership fees.

第7条
甲が乙の所有する器物および施設に対して、故意に損害を与えてはならない。もし損害を与えた場合は、甲は乙の請求する損害賠償を受け入れる。
You promise you will not willfully damage the equipment or facilities provided by Oga Dojo. If you do, you agree to pay compensation assessed by Oga Dojo for the damage you cause.

第8条
乙に関する施設、行事、練習および試合で起きた怪我や事故について、甲は乙に対して一切の保障を請求しない。
You accept all risk of harm or injury while visiting Oga Dojo facilities, training with Oga Dojo, competing in a tournament​, or otherwise participating in an Oga Dojo-affiliated activity and agree to hold Oga Dojo harmless for any damages caused thereby.

第9条
甲の転居、転職、進学のために入会申込時に申請した内容に変更があった場合、甲は乙に対して、すみやかに届け出なければならない。
You promise to notify Oga Dojo promptly if the information listed on your application changes (such as in the event you change residence, change jobs, or enter school).
